All band and color guard students are required to have completed physicals like athletics beginning 8/1/2021.  This is UIL / State policy.  Any student without this on file will not be able to participate in band.

Here are your options:​

Option 1: On your own

i.Arrange an appointment at your own physician/clinic.

ii. Must be completed and to the band office prior to 8/1.

iii. Use required Medical History Form (Attached)


Option 2: Athletics Department

i. If your student is also in a sport that requires a physical, you may use the same physical for band.

ii. Please notify Mr. Vaden at the CHS Band of this.

Option 3: At Band Registration

i. TPC Family Medical will be at our band registration on July 31 to provide Physicals

ii. There will be a $25 fee for this.
